We recommend that you fill the following questionnaire after calling us to schedule your initial appointment. This will save you time and hasten your services as you will not need to fill this out in the office. Of course, all your information is considered confidential and will be entered in your permanent patient file.
We use secure file transfer procedures but if you still feel uncomfortable about e-mailing your information, then feel free to fill the questionnaire then simply print it out and bring those sheets with you to the office.
