To better assist you in understanding your insurance, please bring your medical and dental information on your first visit to us. We work hand in hand with you to maximize your insurance reimbursement and advise you on the covered procedures that your plan offers.

Often, some procedures may be covered only by your dental and/or sometimes only by your medical plan. In any event, we will make certain that you are fully informed regarding your coverage.

Although, we may not participate with certain plans we can however on a case- by- case basis submit a predetermination request to your insurance carrier for authorization. Our office policy is to collect payment for the consultation and records at the time of your visit. We will submit your claim to your insurance company and reimburse you once payment is received. We process reimbursement checks on a monthly basis. Our office requires full payment of the charges not covered by your insurance two weeks prior to your surgery.
